How to Apply
Prior to scheduling your appointment, please review and prepare the required materials below. Without correct documentation and fees, we will not be able to process your application and you will need to schedule a new appointment.
What to Prepare
Completed Passport Application (DS-11). Please print and bring the completed form to your appointment. One DS-11 form is required for each applicant. Do not sign your application. Applications must be signed in person when directed by a passport agent. Use this form if:
- You are applying for the first time.
- Your previous passport was a 5-year passport.
- You do not meet the requirements to renew your passport.
A new passport photo is required for each application.
Passport photo services are available at our offices. Must be paid via Credit/Debit Card as part of Execution Fee.
You may take your own U.S. passport photo as long as it meets the requirements set by the U.S. Department of State.
Both adult and child applicants must submit a passport photo that abides by the following photo guidelines.
- Photo was taken in the last 6 months.
- Clear, in color, and properly exposed with no shadows.
- Background is uniform, plain white, and free of shadows.
- Face is unobstructed – adjust hair so the entire face is visible.
- Remove glasses and hats – ensure that eyes are clearly visible.
- Applicant is directly facing the camera with a neutral expression.
Applicants must bring the original and a photocopy (front and back) of Proof of United States Citizenship and Primary Identity Verification.
Photocopy services are available onsite for a cost of $1 per copy. Must be paid via Credit/Debit Card as part of Execution Fee.
Proof of United States Citizenship
Applicant is born in the United States
Bring one of the following:
- Fully-valid, undamaged U.S. passport (can be expired)
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists applicant’s full name, date of birth, and place of birth
- Lists parent(s)’ full names
- Has the signature of the city, county, or state registrar
- Has the date filed with registrar’s office (must be within one year of birth)
- Has the seal of issuing authority
Applicant is born outside of the United States
Bring one of the following:
- Fully-valid undamaged U.S passport (can be expired)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship
Primary Identity Verification
Bring one of the following:
- Valid, undamaged U.S. passport book or passport card
- In-state, fully valid driver’s license
- Certificate of Naturalization
- Certificate of Citizenship
- U.S. military or military dependent ID
- Current (valid) foreign passport
- Matricula Consular (Mexican Consular ID)
- U.S. Permanent Resident Card (Green Card)
- Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
- Enhanced Tribal Cards and Native American tribal photo IDs
If you do not have a primary identification, a combination of secondary identification documents may be submitted. Final approval of identification documents will be determined by the U.S. Department of State Passport Agency.
Application fee and execution fee are two separate payments. Please do not combine these payments.
Application Fee
This payment must be made payable to the U.S. Department of State. Only the following payment methods are accepted for the application fee:
- Personal check
- Money order
- Cashier’s check
Execution Fee
This payment is separate from the application fee. Credit/Debit card is the only accepted payment method.

For expedited processing, include an additional $60 in your application fee payment.

Reporting your Lost or Stolen Passport
If you need to report that your passport is lost or has been stolen. Submit this along with a DS-11 form to apply for a new passport.

Parental Consent for Applicants Under 16
Minors must appear in person to submit their application.
Both parents or legal guardians must authorize the issuance of the child’s passport.
Submit the following if:
Both parents or legal guardians appear.
- Evidence of child’s relationship to the parents/guardian(s).
- Both the original and photocopy of parent/guardian identification (front and back).
Only one parent or legal guardian appears.
- Evidence of sole parental authority
(or)
- Both the original and photocopy of parent/guardian government-issued identification (front and back).
- DS-3053 or DS-5525
Both parents or legal guardians are unable to appear.
- Notarized statement from both parents or legal guardians granted a third party permission to apply for the child.
- Photocopy of the parent’s or legal guardian’s identification (front and back).
- DS-3053 or DS-5525
Use this form if:
One parent or guardian cannot go with the child to apply in person for the child’s passport. This form is to be submitted in addition to child’s passport application (DS-11).
Consent to Issue Child Passport (DS-3053)
One parent or guardian cannot locate the other parent or guardian. This form is to be submitted in addition to the child’s passport application (DS-11).

Parental Consent for Applicants Age 16-17
Minors must appear in person to submit their application.
Applicants 16-17 must show that at least one parent or guardian knows that they are applying for a passport. Parental awareness may be shown through one of the following methods:
- A parent or legal guardian appears with the applicant. The passport agent accepting the application will ask the parent or legal guardian to sign Form DS-11. Bring a photocopy of that parent or guardian’s ID.
- A signed statement consenting to issuance of a passport from at least one parent or legal guardian. The statement should be accompanied by a photocopy of that parent or guardian’s ID.
